Upon receipt of a Physician's Statement verifying that any student
enrolled in the Burlington Public Schools or placed by the Burlington
Public Schools in a private setting must remain at home or in a hospital
for medical reasons and for a period of not less than fourteen school
days in any school year, the Pupil Services Office shall arrange for
the provision of educational services. The education services will
be provided with Burlington Tutors or by tutors provided in the hospital.
Burlington Public Schools provides a student receiving a publicly
funded education with the opportunity to make educational progress
when a physician determines that the student is physically unable
to attend school. Burlington Public Schools provides the instruction
necessary to enable the student to keep up with his/her course of
study in order to minimize the educational loss that might occur during
the time in which the student is physically unable to attend school
as documented in the Physician's Statement.
Once the student's physician (for example, a pediatrician, internist,
medical specialist or psychiatrist) determines that a student's medical
condition will require either hospitalization or home care for not
less than 14 school days, the parents/guardians must have the physician
complete the Physician's Statement (located on this web site). The
Physician's Statement consists of: the date the student was admitted
to a hospital or confined to the home; the medical reasons for the
confinement; the expected duration of the confinement; and what medical
needs of the student should be considered in planning the home/hospital
educational services. The parents/guardians forward the completed
Physician's Statement to the Pupil Services Office. The Pupil Services
Office will arrange for the home/hospital instruction. Home/Hospital
instruction is typically one to one or small group. Service delivery,
including the time of day that the services are delivered, is determined
in consideration of the medical circumstances of the student. Home/hospital
instruction is coordinated with the instructional content, approach
and student progress with the student's teachers.
Burlington Public Schools requires that an adult is present during
homebound instruction and takes reasonable steps to schedule the instruction
at a time when an adult would be home. Burlington Public Schools ensures
that the student receives the homebound instruction while satisfying
legitimate concerns about safety and liability. The student's teachers
continue to grade the student's work and provide the work for the
homebound instruction.
